Background
Immune checkpoint inhibitors are now backbone therapy for several malignancies. Kidney immune-related adverse events, most commonly AKI from acute interstitial nephritis but also glomerular disease and electrolyte disturbances, complicate their use as prescribing expands.

The statement summarizes the incidence and risk factors, pathophysiologic mechanisms, and clinicopathologic features of ICI-AKI, reviews novel diagnostic approaches and biomarkers, and offers expert-consensus practice points for initial assessment, immunosuppressive management, and consideration of ICI rechallenge after an AKI episode.
Clinical implications
It gives nephrologists and oncologists a shared, practical framework for managing ICI-AKI, addresses special populations such as kidney transplant recipients, and identifies priority areas for future research where high-quality evidence is still lacking.
