Marriage, intimacy, contraception, and a planned pregnancy after a kidney transplant — what is safe, what to adjust, and when to start trying.
Key Points
• Wait at least one year of a stable graft before trying to conceive — creatinine steady, no recent rejection, no active infection.
• Stop mycophenolate (CellCept / Myfortic) and sirolimus (Rapamune) at least six weeks before you try to conceive and switch to Imuran (azathioprine) — never stop them without a replacement.
• Stop the blood-pressure pills whose names end in -pril or -sartan before you try to conceive; switch to labetalol, methyldopa, or long-acting nifedipine.
• Start folic acid 5 mg daily three months before you try to conceive, and keep it through the first trimester.
• Breastfeeding is safe on Prograf, Neoral, Imuran, and a low daily dose of prednisolone — only mycophenolate and sirolimus need to be avoided while you nurse.
• Your tacrolimus or cyclosporine dose usually needs to increase by twenty to twenty-five percent during pregnancy because pregnancy lowers the level in your blood — this is expected, not rejection.
• Take a low-dose baby-aspirin (75 to 150 mg) every day from week twelve of pregnancy until close to delivery to lower the risk of pre-eclampsia.
• Long-acting reversible contraceptives — the hormonal coil, the copper coil, and the implant under the skin — are the first-line choice; condoms alone are less reliable.

• أوقفي أدوية الضغط التي تنتهي أسماؤها بـ بريل أو سارتان قبل محاولة الحمل, وحوّلي إلى اللابيتالول أو الميثيل دوبا أو النيفيديبين طويل المفعول.
• ابدئي حمض الفوليك بجرعة خمسة ملج يوميا قبل محاولة الحمل بثلاثة أشهر, واستمري عليه طوال الثلث الأول.
• الرضاعة الطبيعية آمنة مع البروجراف والنيورال والإميوران والكورتيزون بالجرعة المعتادة - الدواءان الوحيدان اللذان نطلب تجنبهما أثناء الرضاعة هما السل سبت والراباميون.
• جرعة البروجراف أو النيورال ترتفع عادة بنسبة عشرين إلى خمسة وعشرين بالمئة أثناء الحمل لأن الحمل يخفض مستواها في الدم - وهذا أمر متوقع ولا يعني الرفض.
• تناولي الأسبرين بجرعة قليلة (من خمسة وسبعين إلى مئة وخمسين ملج) يوميا من الأسبوع الثاني عشر للحمل وحتى قرب الولادة لتقليل خطر تسمم الحمل.
• الوسائل طويلة الأمد لمنع الحمل - اللولب الهرموني واللولب النحاسي والغُرسة تحت جلد الذراع - هي الخيار الأول؛ الواقي الذكري وحده أقل فعالية.
