Diet After Transplant

What to eat and what to leave out after a kidney transplant — salt by the teaspoonful, foods that change your medicine levels, Ramadan fasting by risk group, weight, and the small daily statin we now keep every adult transplant recipient on.

Key Points

• Aim for no more than one level teaspoonful of table salt across the whole day — about five grams of salt or two grams of sodium. Most of that salt is already inside bread, cheese, processed meats, and salty sauces, not in the salt shaker.
• Drink two to three litres of fluid a day unless your team has told you otherwise — pale-yellow urine is the simplest sign that you are drinking enough.
• Avoid grapefruit and grapefruit juice, Seville (bitter) orange, and large daily amounts of pomegranate juice — they raise the level of Prograf or Neoral in your blood. Star fruit can directly damage a transplanted kidney and must be avoided completely.
• Stick to safer fruit: apples, bananas, watermelon, peaches, oranges (sweet), dates in moderation.
• Aim for a waist size under 102 cm for men and under 88 cm for women. Swap white rice and white bread for whole-grain versions and replace sugary drinks with water flavoured with mint or lemon.
• Every adult transplant recipient is started on a small daily statin — atorvastatin (Lipitor) or rosuvastatin (Crestor) — even if cholesterol numbers look normal, because heart trouble is the leading long-term risk after a transplant.
• Whether you can fast in Ramadan is a risk-based decision your team makes with you: very high risk and high risk groups should not fast; moderate risk is case-by-case; low risk can fast with daily check-ins.
• Cook meat, chicken, fish, and eggs all the way through. Avoid raw or undercooked meat and fish, unpasteurised dairy, soft cheeses, and street food.

• اشرب من لترين إلى ثلاثة لترات من السوائل يوميا ما لم يخبرك فريقك بخلاف ذلك - ولون البول الأصفر الفاتح هو أبسط دليل على أنك تشرب ما يكفي.
• تجنب الجريب فروت وعصيره, والبرتقال المر (السيفيلي), وكميات كبيرة يومية من عصير الرمان - فهي ترفع مستوى البروجراف أو النيورال في الدم. وفاكهة النجمة (الكارامبولا) قد تضر الكلية المزروعة مباشرة ويجب تجنبها تماما.
• اعتمد على الفواكه الآمنة: التفاح والموز والبطيخ والخوخ والبرتقال الحلو والتمر باعتدال.
• اهدف لمحيط خصر أقل من ١٠٢ سم للرجل وأقل من ٨٨ سم للمرأة. استبدل الأرز والخبز الأبيض بنسخهما السمراء, واستبدل المشروبات السكرية بالماء بالنعناع أو الليمون.
• يبدأ كل بالغ بعد زراعة الكلى على جرعة قليلة من الستاتين يوميا - أتورفاستاتين (الليبيتور) أو روزوفاستاتين (الكريستور) - حتى لو كان الكولسترول طبيعيا في التحليل, لأن مشاكل القلب هي أكبر خطر على المدى الطويل بعد الزراعة.
• هل تصوم رمضان أم لا قرار حسب درجة الخطر يأخذه فريقك معك: الخطر العالي جدا والخطر العالي لا يصومان؛ الخطر المتوسط حالة بحالة؛ الخطر المنخفض يصوم بمتابعة يومية.
• اطبخ اللحم والدجاج والسمك والبيض جيدا. تجنب اللحوم والأسماك النيئة, ومنتجات الألبان غير المبسترة, والأجبان الطرية, وأكل الشارع.
