Plant-dominant low-protein diet versus standard care in adults with chronic kidney disease stages 3-5: A randomized controlled trial protocol.

This Lebanon-based RCT protocol will test whether a plant-dominant low-protein diet (PLADO) improves metabolic acidosis and other outcomes compared with standard renal diet counseling in 48 adults with CKD stages 3-5; results are not yet available.

Background

The plant-dominant low-protein diet (PLADO, 0.6-0.8 g/kg/d protein, at least 50% plant-derived) may improve acid-base and metabolic outcomes in CKD, but evidence on safety and feasibility is limited, especially in resource-limited settings.

Study design

48 adults with CKD stages 3-5 at a Lebanese medical center will be randomized 1:1 to PLADO or standard-of-care renal diet for 6 months plus a 3-month free-living phase, with monthly individualized dietary counseling.

Clinical implications

The primary outcome is change in plasma total CO2 (metabolic acidosis) at 6 months; secondary outcomes include uremia and anemia markers, nutritional status, quality of life, gut microbiota composition, and cost-effectiveness, relevant to CKD dietary practice in economically constrained health systems.
