KDOQI US Commentary on the 2021 KDIGO Clinical Practice Guideline for the Management of Glomerular Diseases.

The National Kidney Foundation's KDOQI work group issued a chapter-by-chapter US commentary on the KDIGO 2021 Glomerular Diseases guideline, broadly endorsing it while flagging areas where US practice and remaining evidence gaps call for nuance.

Background

The KDIGO 2021 Clinical Practice Guideline for the Management of Glomerular Diseases was the first update to these recommendations since 2012. The intervening years brought a faster pace of molecular understanding of glomerular disease and the introduction of newer immunosuppressive and targeted therapies, making an update necessary. Despite the update, many areas of controversy remained, and further developments occurred after publication that the guideline did not capture.

Key recommendations

The KDOQI work group generated a companion opinion article that provides commentary specific to implementing the KDIGO 2021 guideline within the United States. The commentary follows the guideline chapter by chapter, covering the range of glomerular diseases addressed by KDIGO, including IgA nephropathy, membranous nephropathy, minimal change disease, focal segmental glomerulosclerosis, lupus nephritis, ANCA-associated vasculitis, and complement-mediated diseases.

Clinical implications

The commentary is intended to help US clinicians interpret and apply the KDIGO 2021 recommendations within the context of the US health care system. By offering implementation-focused perspective and highlighting persisting controversies and knowledge gaps, it serves as a practical companion to the international guideline for nephrologists managing patients with glomerular disease.
