KDIGO 2024 Clinical Practice Guideline for the Evaluation and Management of Chronic Kidney Disease in Children and Adults: a commentary from the European Renal Best Practice (ERBP).

A European Renal Best Practice (ERBP) commentary on the KDIGO 2024 CKD guideline's evaluation and risk-assessment sections concludes that CKD diagnosis and treatment remain poor across Europe, and frames the guideline as an urgent call to action to improve care for children and adults with CKD.

Background

The KDIGO 2024 Guideline for the evaluation and management of chronic kidney disease (CKD) arrives 12 years after the paradigm-changing 2012 guidelines. The authors describe an unprecedented era in nephrology, with sodium-glucose cotransporter-2 inhibitors, glucagon-like peptide-1 receptor agonists and non-steroidal mineralocorticoid receptor antagonists now proven in multiple randomized controlled trials to reduce both CKD progression and cardiovascular morbidity and mortality.

Study design

This is an expert commentary from the European Renal Best Practice (ERBP) group, not an original study. It reviews the KDIGO 2024 guideline sections covering the evaluation and risk assessment of individuals with CKD from a European perspective. The guideline itself is aimed at a broad audience caring for children and adults with CKD and offers practical, actionable steps to improve care.

Key findings

The authors note that, although the previous guideline was published 12 years ago and CKD assessment has been emphasized by many national and international nephrology, cardiology and diabetology guidelines and societies, the diagnosis and treatment of CKD remain poor across Europe.

Clinical implications

ERBP positions the KDIGO 2024 CKD guideline as an urgent call to action to improve the diagnosis and care of children and adults with CKD across Europe. In the authors' words, the knowledge of what needs to be done already exists; the priority now is implementation.
