Interventions for focal segmental glomerulosclerosis in adults.

This Cochrane review of 15 trials (560 participants) found only low-certainty evidence in adult steroid-resistant FSGS; cyclosporin for at least six months may raise the chance of remission.

Study design

Systematic review of 15 RCTs/quasi-RCTs (560 participants) of immunosuppressive and non-immunosuppressive treatments in adults with FSGS.

Key findings

Pooled cyclosporin data (4 studies, 231 participants) suggested higher complete remission (RR 2.31, 95% CI 1.13 to 4.73; low certainty) and complete or partial remission (RR 1.64, 95% CI 1.10 to 2.44). MMF versus prednisone made little or no difference, and whether sparsentan lowers proteinuria more than irbesartan is uncertain.

Clinical implications

Authors call for clearly defined populations (primary, genetic, secondary FSGS) in future trials.
