Executive summary of the KDIGO 2024 Clinical Practice Guideline for the Management of Lupus Nephritis.

The KDIGO 2024 guideline is a focused update of the lupus nephritis chapter prompted by the regulatory approval of two novel drugs, providing revised, evidence-graded guidance for treatment.

Background

The KDIGO Clinical Practice Guideline for the Management of Glomerular Diseases was published in 2021. Since then the pace of drug development has accelerated, driven in large part by rapidly accumulating insights into disease pathogenesis from genetic and molecular studies. KDIGO committed to providing periodic updates as new developments emerged for each disease.

Key recommendations

After the 2021 guideline, two novel drugs received regulatory approval for the management of lupus nephritis, prompting this first KDIGO guideline update. The executive summary provides a quick-reference overview of the most important changes from the lupus nephritis chapter. The update incorporated evidence from randomized controlled trials published since February 2022 and followed the GRADE approach to appraise the strength of recommendations and certainty of evidence.

Clinical implications

The update is focused on guidance related to treatment of lupus nephritis, including initial and subsequent immunosuppressive therapy, and follows the established chapter template covering diagnosis, treatment, and special situations. Limitations of the current evidence are discussed and areas for future research are highlighted to guide clinicians caring for patients with lupus nephritis.
