Clinical practice guideline for the prevention and management of peritoneal dialysis associated infections in children: 2024 update.
The 2024 update of the international clinical practice guideline for prevention and management of peritoneal dialysis (PD)-associated infections in children provides updated, evidence-graded guidance on antibiotic therapy, antibiotic stewardship, and new clinical benchmarks, reflecting evolving antibiotic susceptibilities and pediatric registry data.
Background
Infection-related complications remain the most significant cause of morbidity and technique failure in infants, children, and adolescents receiving maintenance peritoneal dialysis. This 2024 update builds upon prior guidelines published in 2000 and 2012.
Key recommendations
Developed by an international group of pediatric PD experts based on a review of published literature and pediatric PD registry data, the guideline prioritized updating key clinical issues from the 2012 version and addressed additional questions using the PICO format. New guideline statements were highlighted by those pertaining to antibiotic therapy of peritonitis, driven by the evolution of antibiotic susceptibilities, antibiotic stewardship principles, and clinical registry data, alongside new clinical benchmarks.
Clinical implications
The comprehensive treatment guidance is intended to standardize prevention and management of PD-associated infections in children and to reduce morbidity and technique failure. The workgroup also outlined recommendations for future research designed to fill important knowledge gaps in pediatric PD infection care.
