Follow-Up Visits
Why follow-up after a kidney transplant never really ends, what happens at each visit, and the home checks that protect the kidney between visits.
Key Points
• Follow-up is closest in the first year — two to three visits a week in the first fortnight, then weekly, then every two weeks, then monthly through the rest of year one.
• Buy an upper-arm blood-pressure cuff for home use; check at least twice a week in the early months and weekly long-term, target around 130 over 80, and bring the log to every visit.
• Every visit checks weight, blood pressure, kidney function, drug level, blood sugar, and screens for silent infections — most problems give no symptoms until they are far along.
• Some patients develop high blood sugar in the months after a transplant even with no family history — the team checks for this at every visit and acts early if it rises.
• In the first year the team checks a special blood test for BK virus every few months; if it is detected the visits become more frequent for a while until it settles — nothing extra is asked of you.
• From year two onward visits are every two to three months, and long-term every three to six months — but they never stop.
• Never miss two visits in a row, never stop a medicine on your own, and bring your full medicine list and home blood-pressure log to every appointment.

• كل زيارة تشمل الوزن والضغط ووظائف الكلى ومستوى الدواء والسكر وفحوصات الالتهابات الصامتة - فمعظم المشاكل لا تظهر أعراضها إلا متأخرة.
• بعض المرضى يرتفع لديهم السكر في الأشهر التي تلي الزراعة حتى دون تاريخ عائلي للسكر - والفريق يفحصه في كل زيارة ويتصرف مبكرا إذا ارتفع.
• في السنة الأولى يقوم الفريق بتحليل دم خاص لفيروس BK كل بضعة أشهر, وإذا ظهر تصبح الزيارات أكثر تواترا لفترة حتى يهدأ - ولا يطلب منك شيء إضافي.
• من السنة الثانية تصبح الزيارات كل شهرين إلى ثلاثة, وعلى المدى البعيد كل ثلاثة إلى ستة أشهر - ولكنها لا تتوقف أبدا.
• لا تفوّت زيارتين متتاليتين أبدا, ولا توقف دواء من تلقاء نفسك, وأحضر قائمة أدويتك الكاملة وسجل قياسات الضغط في كل موعد.
