- What tacrolimus trough should I target in the first months after transplant?
- Early after transplant, higher troughs of roughly 6 to 10 ng per mL are typical, tailored to immunologic risk, then tapered toward 4 to 7 ng per mL for maintenance. Lower the target when BK viremia appears; hold it higher for higher-immunologic-risk recipients. Always interpret the trough alongside the full immunosuppression regimen.
