When to use it
ESA initiation and titration require dialysis status, product, current dose frequency, the time between hemoglobin results and individual risks. These inputs cannot support a dose change. KDIGO 2026 recommends an adult ESA hemoglobin target below 11.5 g/dL; correct reversible causes and use the product label and prescribed protocol.
